Premier Choice, LLC is a Service Coordination Agency that is approved by the Office of Long Term Living (OLTL). Our Service Coordinators (SC) do the following to assure the quality of home and community-based programs:
Evaluation: Conduct an accurate evaluation every year of your strengths, needs, preferences, supports, and desired outcomes.
Service Plan Development: Work with you to design a service plan that makes it possible for you to meet your goals.
Referral: Provide information to help you choose qualified providers and make arrangement to assure providers follow the service plan.
Monitoring: Ensure that you get authorized services and that these services meet your identified needs and goals. Monitor participant health and safety.
Problem Solving: Support you in solving problems when something goes wrong as well as help prevent potential problems.
